Individual Appointments for Diabetes

One-on-one consultations are available for people who are unable to fit one of the classes into their schedule, have been to class previously, or who need assistance in a specific area of diabetes.

 

Individual sessions are available for:

 

  • Diabetes management to learn skills to manage blood sugars, including:  prevention and treatment of high and low blood sugars, medication and effect on blood sugar, insulin administration, blood glucose monitoring and use of results
  • Nutrition to learn the effect of various foods on blood sugar, amount of carbohydrate needed for your height and weight, carbohydrate content of foods, reading labels, and eating out
  • Intensive Management to learn how to maintain blood sugars in target range with multiple daily injections.  Techniques based on your personal insulin sensitivity are provided in order for the user to adjust insulin according to current blood sugar, carbohydrate content of the meal and activity level.  The goal is for the person with diabetes to live a normal life while achieving blood sugar control.  

Follow-up appointments are recommended to see how you are doing and to answer any questions you may have.  Quarterly update sessions are provided to give you the support that you need to stay on track.

INDIVIDUAL CONSULTS are ONE HOUR each and are scheduled with a Diabetes Nurse Educator and/or a Nutritionist.

Glucose Meter, Blood Sugar RecordsPrescription for a Glucose Meter, Referral Form from your doctor, List of any Medications you are taking with the dosage and time, Health Questionnaire, Insurance Cards, we will file charges with your insurance.  It is important that you check with your insurance company and ask what they cover for Diabetes Education CPT Code G0109.

 

We do not encourage the presence of children. We have staff and other patients with allergies, so please refrain from wearing perfume. You may bring a sweater or light jacket, the classroom is sometimes cool.
