Patient Privacy
Your private health information is protected by law. All healthcare providers are required to follow the requirements of the Health Insurance Portability and Accountability Act (HIPAA), as well as other Federal and State regulations and their interpretive guidelines.
When you are admitted to Providence Hospital (inpatient or outpatient), you will receive a copy of our Notice of Privacy Practices. This outlines how your private health information can be used, who will have access to it and your rights and responsibilities concerning your protected health information.
Patients in the hospital often want friends and family members to be able to find them or get information about them. Providence Hospital is allowed to include certain limited information about you in our patient directory. This information may include your name, your location in the Hospital, your general condition (e.g., fair, stable, etc.) and your religious affiliation.
The patient directory information, except for your religious affiliation, may also be released to people who ask for you by name. Your religious affiliation may be given to a member of the clergy, such as a priest or minister, even if they do not ask for you by name.
Please be sure to let your family and friends know to ask for you by your first and last name. If you use a nickname or middle name as your first name, be sure to let people know that you will be registered under your legal name.
If you do not wish to be included in the facility directory, please notify us at the time of admission.
